[XERCESC-774] Output with LocalFileFormatTarget fails silently

Description

If a LocalFileFormatTarget cannot create/write to it's file, there is no way for
the caller to discover the failure.

Expected behaviour: if output is unsuccessful the caller should be notified,
preferably by throwing an exception in the constructor of LocalFileFormatTarget
(but it may be necessary also in writeChars.)
