[UIMA-252] bold italics highlighting does not work in code sections of the html documentation

Description

The bold italics highlighting in code sections does not work in HTML document. The same section in the pdf documentation looks fine.

E.g. search for "CATALINA" in the tutorials_and_users_guides and see the next xml fragment code section. Within this section there should be some
highlights but they are not displayed correctly in the HTML document.
