[SOLR-13144] Reference guide section on IndexUpgraderTool needs to be updated

Description

The section on IndexUpgrader Tool (and I'll remove the space too) leads to the conclusion that you can successfully upgrade an index X->X+1->X+2 which isn't true. I'll attach a proposed doc change in a minute.

I'm thinking of only putting this in master and 8x on the theory that AFAIK, the enforcement of this restriction is only 8x and later.

This is doc-only, if no objections I'll commit this weekend at the latest.
