[OLIO-25] geocoder instructions and URL incorrect

Description

The geocoder source code in the repository does not match the instructions in docs/setup_php.html. The doc was written before the source code was checked in.
Also, the URL for the geocoder in the the etc/config.php file of the php app directory is incorrect. It should be http://GEOCODER_HOST:8080/geocoder/geocode
