Update version in documents

• `elasticsearch:2.4.6` will be replaced in documentation

Update `upgrade-instructions.md`

There a upgrading note from ES 1 -> 5 here

And ES 5 -> 6 here. Describe the upgrade & benefit from the upgrade.

You will RUN the migration on top of a debian instance to experiment with this process and validate it.
