[HBASE-5280] Remove AssignmentManager#clearRegionFromTransition and replace with assignmentManager#regionOffline

Description

These two methods are essentially the same and both present in the code base. It was suggested in the review for HBASE-5128 to remove #clearRegionFromTransition in favor of #regionOffline (HBASE-5128 deprecates this method, but it is internal to the HMaster, so should be safely removable from 0.92 and 0.94).
