Description

Current 1.6 view server error messages look like:

[Mon, 09 Mar 2015 02:29:04 GMT] [info] [<0.25727.141>] OS Process #Port<0.3191211> Log :: function raised exception (new TypeError("doc.attr is undefined", "undefined", 3)) with doc._id some_doc_id

This doesn't say which DB, design doc, or view is causing the issue, which makes it very difficult to debug. Please add those three items to the log message.
