Description

When FailureDetector is called to check an IP which is no longer part of the cluster, the error message is unhelpful:

FailureDetector.java:223 - unknown endpoint /10.1.2.3

We should print additional information to make it easier for users to determine where in the stack the "unknown IP" is coming from.
