camel-fhir: Integration tests are slow to execute

    Description

      The FHIR integration tests are quite slow and it's due to the test infra mock HAPI server container being stopped / started on every integration test class. We should be able to start the container once and reuse it for every itest (I tried but did not figure out how to do it).
