Support commit/rollback behind "dumb" loadbalancer

    Description

      Some follow-on work that I punted on in CALCITE-767 for commit/rollback functionality.

      We've been trying to keep the Avatica driver functional when sitting behind a generic HTTP load balancer which won't guarantee clients always return to the same backend server.

      jamestaylor is working on some Phoenix-layer changes that would enable this in PHOENIX-2411, but we'll likely have to make some RPC-level changes for Avatica to help enable Phoenix.
