[AXISCPP-799] Elemenate the need for operationRequestMap in server.wsdd

Description

The operationRequestMap parameter was added to the server.wsdd to help drop the need for SOAPAction HTTP header.

However, it is felt that this is less user friendly to add stuff to the server.wsdd.

Hence we should drop the need for operationRequestMap and try to resolve this with the help of the server wrapper as John has suggested.
