[AMBARI-6357] deleted host can not be added properly again

Description

1. pick a host with ganglia, dn, nm on it
2. stop all host components
3. put in mm
4. delete host
5. re-add host
6. during add host wizard, it shows DN and NM checkbox already checked and you are unable to uncheck
7. it only installs + starts ganglia monitor
8. after completion, when you get back into hosts page, the host only has ganglia monitor, no DN or NM.

• should have option to re-add (or not) DN and NM in step #6
• based on what i chose, should have installed in step #7
