I reduce this one to the "built in heading duplicated" problem with "TOC-MarkText-CustomHeadingStyles-1.doc"

Steps how to reproduce Reproducible with server installation of "AOO 4.1.0-Dev – German  UI / German locale - [AOO410m1(Build:9750)  -  Rev. 1537973 - 2013-11-03]" on German WIN7 Home Premium (64bit)", own separate user profile:

0. Download / Unzip Attachment 81858 [details] for Bug 123602
1. From AOO start Center open "TOC-MarkText-CustomHeadingStyles-1.doc"
   > TOC looks fine
2. On TOC 'Rightclick -> Update'
   Bug: all items "Built-in heading " for page 5 become duplicated    :-(
3. On TOC 'Rightclick -> Edit Indix/Table -> Index/Table'
4. Uncheck "Additional Styles"
   > Looks fine again

(a) Root for the problem seemsto be "Additional Styles", currently I can't tell 
    whether the main problem is that 
(a1) 'Rightclick -> Edit Indix/Table -> 
     Index/Table - Additional Styles' is checked in the imported .doc 
     or 
(a2) that "Custom Heading 4" is activated in the styles selector in 
     "Additional Styles"
     My suspect: (a1)
(b) not a new problem, already reproducible with AOO 3.4.0, OOo 3.1.1

(c) Already reproducible with  Pre-3.4.0 (OOo 3.1.1), but because of crippled 
    Version selector (Bug 123063) no useful info can be left
(d) Latest confirmation for AOO 4.1.0-dev, but because of incomplete 
    LCo selector (Bug 123063) no correct information can be left.
	
(e) This one looks similar to 
    "Bug 71231 - Table of Contents Additional Styles duplicates", but
    strange. OOo 2.0.2 reproduces Bug 71231, but not this one here (although
     'Additional Styles' activated for both sample documents). 
    AOO 4.1.0-dev reproduces problems in both sample documents.
    So it seems there have been changes in between

Can you please submit separate Bug reports (with reference to the test kit here) for the other problems you observed?

Seems NOT.
I did some quick tests with server installation of "AOO 4.1.0-Dev – English  UI / English locale - [AOO410m1(Build:9750)  -  Rev. 1537973 - 2013-11-03]" on German WIN7 Home Premium (64bit)", own separate user profile. UPDATE ALL results for different documents:

(1) is this report
(2) reproducible, new Bug report soon, might be related to (6)
(3) looks like DUP of (1)
(5) reproducible, new Bug report soon, might be related to (2)
(6) reproducible, Bug 123697 - In particular.doc in TOC bullets disappear 
    after "Update All"
