I see some evidence that we have a user error here, but let's see.

Thank you for your feedback – unfortunately important information we will 
need to reproduce your problem is missing.
Please add all information requested in following:
a) Write a meaningful Summary describing exactly what the problem is
b) Attach a sample document (not only screenshot) or refer to an existing 
  sample document in an other Bug with a link; to attach a file to this 
  bug report, just click on "Add an attachment" right on this page.
c) Contribute a document related step by step instruction containing every 
  key press and every mouse click how to reproduce your problem 
d) Attach screenshots with comments if you believe that that might explain the 
  problem better than a text comment. Best way is to insert your screenshots
  into a DRAW document and to add comments that explain what you want to show
e) if possible contribute an instruction how to create a sample document 
  from the scratch
f) add information 
  f1) what EXACTLY is unexpected
  f2) and WHY do you believe it's unexpected (cite Help or Documentation!)
  f3) concerning your PC (video card, ...)
  f4) concerning your Operating System (Version, Distribution, Language)
  f5) concerning your AOO version (with Build/revision ID if it's not a 
     public release) and localization (UI language, Locale setting)
  f6) AOO settings that might be related to your problems 
  f7) how you launch AOO and how you opened the sample document and
      how you proceeded e-mail with what e-mail-client
  f8) Whether your problem persists when you renamed your user profile 
     (Quit Quickstart before!) before you launch AOO (please see
     <http://www.openoffice.org/development/releases/3.4.1.html#AOO3.4.1ReleaseNotes-KnownIssues>)
  f9) Whether that worked in former OOO / AOO Versions

Reporter sent a test kit to me (I asked him to attach it here), and if I understand everything correctly problem is  Reproducible with "AOO 4.0.1   – German UI / German locale  [Rev. 1524958 2013-09-20 11:40:29]" on  German WIN7 Home Premium (64bit)", “historic”  4.0  User Profile used for all  predecessor versions:

1. From AOO Start Center open his "overzicht IK tem dec 2012.docx"
2. Open a compy of that document with MS WORD viewer and compare:
   In MS WORD viewer there is some kind of flow chart 
   Bug: In AOO document looks empty

Some modifications here due to my questions:

(a) see new summary!
   This is a Writer - open-import bug
(b) I hope reporter will attach testkit, soon 
(c) see above!
(d) screenshots not required
(e) Still missing. Philppe Duhamel, please contribute info how you created
    "overzicht IK tem dec 2012.docx" (especially: with what software?)
(f) no more info required except (f9)

f91) Has been fixed in LibO 4.0, may be more early
f92) already broken in OOo 3.1.1

I installed the previous version of OO. Same problem. So OSX 10.9 related.

Sample Document

From reporter's sample (what I received by e-mail) I created a new one what can't be considered confidential. 

For me all relevant behavior was completely the same as with reporter's original sample document

(g) Latest confirmation for AOO 4.1.0-dev, but because of incomplete LCo selector 
    (Bug 123063) no correct information can be left.
