style and class attributes not merged when user-supplied in XHTML5 sink

In some cases our code sets some default style or class if and only if user supplies nothing. Or the other way around, our code is dropped when something user-supplied is passed. This leads to broken output, e.g., MPIR-431 where both text-align and vertical-align need to be present.
