Remove Entity View mode from DocumentAnalyzer

    Description

      The DocumentAnalyzer's entity view mode is currently broken, and it only ever worked for annotators that used an IBM-proprietary type system. We need to remove this mode and leave the ability for IBM to add such capability in its own derivative of the DocumentAnalyzer.
