Various Exceptions on first concurrent request

      Description

      After restarting my webapp, when I try to access a page from 2 differnet browsers concurrently, the second browser will get an exception page with various exceptions.
      With "-Dorg.apache.tapestry.disable-caching=true" I get an error every time a page is accessed concurrently. Sometimes even on both browsers with exceptions from ArrayIndexOutOfBoundsException to NullPointers.

        This may have been fixed with recent changes having to do with async operations on both the client side IO calls and tapestry enhancement workers.
