[STDCXX-437] [MSVC 6.0] remove MSVC 6.0 specific hacks from sources

Description

If we're not going to support MSVC 6.0 we should remove the numerous ugly hacks put in place to accommodate the various bugs in this compiler from stdcxx source code. The cleanup will significantly improve the readability of the code, especially in containers where it will allow us to remove kludges dealing with the compiler's inability to parse member templates defined outside the body of their class.
