[SLIDER-54] Report container hostname and log file location for containers that have failed

Description

When a container fails, it is useful for the application owners to know the host and log locations if they need to debug the failure. This is predicated on yarn to have been configured to retail local logs. If the logs are pushed to a file system such as HDFS then its also useful to report the log location in fs.
