[QPID-7208] Failure to add a certificate to a managed truststore not reported a second time in some browsers

Description

1. Create a Managed Certificate Store
2. Click "Add Certificate" with upload a PEM with a deliberate error in file
3. Error message "422 - java.security.cert.CertificateException"
4. Click Ok
5. Click "Add Certificate" with upload the same PEM
6. On Firefox, error reported again, On Safari, request to upload the file is seemingly ignored

Problem is the file widget is not reporting a change, presumably, because the file has not changed. The file input file should be reset following either a successful upload, or a failure.
