[PDFBOX-4123] PDF will only print rasterized

Description

This PDF will only print rasterized.  It looks fine in Adobe Reader, but once printed using PDFBOX, it will always print rasterized.

Information about how the PDF was created (reported by a 3rd party, can get more details if needed):

• Created with Adobe Illustrator
• Form-filled programmatically using using FPDI
• The PDF before FPDI does not experience this issue.

Also reproduced on CUPS-PDF, Microsoft Print To PDF

The symptom from the business area was "slow printing", but this symptom was a side effect of a high resolution rasterization, causing the PDF to spool to about 800MB.
