[PDFBOX-3817] Signature form names not standard

Description

We are not using the correct form names in our signature classes. The n2 layer is named "n1" in 2.0 and "n0" in 1.8. The n0 layer should be empty. The holding form must be named "FRM" and not "FRM1" or "FRM0", see
