Description

Steps to regenerate:
1) Go to Catalog -> Categories
2) Find a category and go to rollup screen.
3) If there are multiple parent categories under "Category Rollup : Parent Categories" date time field doesn't display calendar icon.

Root Cause:
Multiple date time field with the same id.
