Log Warning:

XmlFileLoader: File file:/.../applications/product/widget/catalog/ProductForms.xml process error. Line: 131. Error message: cvc-complex-type.4: Attribute 'description' must appear on element 'entity-options'.

There are several other places in that XML file and maybe other XML files where the description attribute is missing on entity-options elements.
