[MYFACES-4640] Smaller issues with the appendIussingElements code discovered by Tobago Tests

Description

The Tobago people have discovered several smaller issues regards the new appendIssuingItem code where the item was incorrectly added or with a wrong value. 

This is fixed now and fortified with unit tests testing the scenarii!

Tobago runs now with jsf.js_next_gen@4.0.2-beta.8 so I will move this version back into myfaces!

The fixes from tobago already have pending pull requests for 3.0 and 2.3-next
