[CAUSEWAY-1831] If Accept header bad, then our exception handler throws an NPE and so issue of bad header is not reported.

Description

If RestEasy throws exception due to bad Accept header, this happens before Isis has set up a session.  We therefore need to guard against this in the exception handler to avoid the NPE.
