avoid numbering TOC entry and fix item numbering which has 2 dots

    Description

      as seen in MPDF-59, first TOC entry is the entry for the TOC itself: it should not be numbered, since numbers from next entries are shifted

      while at it, sub-numbers have 2 dots, like "1.1.", "1..2."
