[COUCHDB-742] the link in Futon "Welcome <username>!" is not proxy aware

Description

I'm running CouchDB behind a nginx proxy, under a separate path /couch

After the issue #321 has been closed mostly Futon seems to work, except the link under "Welcome <username>!" (right bottom corner of Futon after you create an admin user).
