Description

Change how ES indexes are exposed in calcite schema

1. Allow multiple indexes to be present in the same schema (current limitation is schema per index)
2. Automatically import existing indexes using ES API (with optional regexp filtering). Elastic has an API listing all indexes / types / mappings
3. By default, adopt table per index approach. ES types
   have been deprecated.
