If you use fo:table-and-caption, the result is just empty space, while
the same table as "stand-alone" table works.

If you check section B4 on the features page:

You'll notice that fo:table-and-caption hasn't been implemented yet ... which,
by the way, I'd like implemented sooner than later for the purpose of converting
some docbook into pdf.
