[MATH-853] "MathRuntimeException" to become the base class for all Commons Math exceptions

Description

From discussions that led to this thread, we concluded that the advantages of having a single hierarchy for all CM expections were more important than the drawbacks.

The base class will be named "MathRuntimeException" (to be located in package "o.a.c.m.exception").

In version 4.0, all CM exceptions will be modified so that they inherit from that class.
