Description

• Breaking out Schema Design chapter into separate file (from book.xml)
• Adding 3 different fairly lengthy schema design cases from common-questions on the dist-list.
• Also re-organized the Schema Smackdown section into this Case Studies section.

Note: I am going to move the existing schema Case Study in the "Case Study" chapter into this section, but in a different ticket. It makes sense to have the schema design case studies in the Schema Design chapter.
